The purpose of this project is to add state-of the-art brain imaging to a double-blind randomized controlled trial of repetitive transcranial magnetic stimulation (rTMS) for the treatment of anhedonia. Our aim is to increase understanding of the neurobiology of anhedonia and the mechanisms underlying the rTMS effect.RTMS is a new therapeutic option for pharmacotherapy resistant anhedonia, diminished pleasure, in patients suffering from schizophrenia or depression. However, the mechanism of action is poorly understood.The effect of rTMS on anhedonia in schizophrenia or depression will be assessed. We will stimulate over a new target, the dorsomedial prefrontal cortex and the underlying anterior cingulate cortex (ACC). Brain imaging with combined positron emission tomography (PET) and magnetic resonance imaging (MRI) will be performed before and after the treatment series on 60 patients. Simultaneous PET/MR will enable assessment of the balance between the main inhibitory and excitatory systems in the brain – the γ-aminobutyric acid (GABA) and glutamate system. We will acquire a unique recording of the interplay between receptors, neurotransmitters, and connectivity of the main anhedonia network nodes; the ACC and the ventral striatum. The brain imaging data will also be correlated to a wealth of psychiatric, cognitive, and neurophysiological assessments.The projectoffers insights that may lead tonew and more effective treatments for these highly disabling disorders.
